What are your office hours?

Office hours are Monday thru Friday, 8 am - 5 pm. We are closed on most federal and some local holidays.

Where are you located?

We are conveniently located on the Windward side of Oahu in Kailua.

Metered parking is available in the parking lot area surrounding our clinic. Be sure to bring coins to feed the meters.

How soon can I be seen?

We try to see patients as soon as possible. Appointments with specific physicians may be subject to schedule and availability.

Our doctors schedules rotate and alternate by clinic location; Please call in advance to schedule your appointment.

What insurances do you accept?

We participate with most commercial insurance carriers, including the following:

Medicare, HMSA PPO, HMSA Akamai Advantage, HMSA Quest, HMSA HMO Castle Health Group, UHA, HMA, HMAA, *MDX (includes Aetna, United Healthcare PPO, United Healthcare Medicare, Humana, Tricare, DMBA), Ohana Medicare, Alohacare, Kaiser Added Choice.

*Please check with your insurance carrier prior to making an appointment to ensure our provider is participating with your plan.

Work Comp/No Fault are accepted on a case by case basis.

We do NOT accept Third Party Liability. 

Do I need a referral?

Self referrals are accepted, however, some insurance plans require a referral and/or authorization from your primary care physician/health plan.  Please check with your insurance carrier regarding any current information regarding your benefits.

For health plans that require a referral and/or authorization, please make sure this has been obtained prior to your visit.

If you are being referred by another physician, please have them forward any pertinent information prior to your visit. 

How do I schedule/cancel an appointment?

Appointments for either location can be made by calling 261-4658.

As a courtesy to all, please call our office 24 hours in advance should you need to cancel and/or reschedule your appointment.

For patients with one or more no-show/cancellations (without a 24 hour notice) – a no-show/cancellation fee may be apply.

What do I need to bring to my appointment?

If you are a new patient, please arrive 20 minutes prior to your appointment time to allow yourself time to complete patient forms and to be processed into our system.

Likewise, if you are an established patient coming in for a new problem (or have not been seen within the past year), please arrive 20 minutes prior to your appointment time, as additional forms may also need to be completed.

In order for us to better serve you, it is important you bring the following items to your visit:

  • Photo identification
  • Your current health insurance card
  • A list of current medications and previous surgery information
  • Any and all previous x-rays, MRI, CT or bone scans relating to your visit
  • Patients under the age of 18 must be accompanied by a parent or legal guardian
  • Translator and/or interpreter, if applicable

Co-payments will be collected at each visit.

Please be sure to notify the staff of any changes.

How long will my appointment take?

Your appointment time allows for one priority problem to be addressed. The length of your visit will depend on the complexity of your case, as well as the complexity of others that are being seen that same day. Expect to spend approximately 1-2 hours on your initial visit.

We apologize in advance for any time you may have to wait to see our physicians as some cases require special attention and/or require x-rays to be taken. However, we will do our best to see you as soon as possible and provide you with individual care and attention.

What is your medication policy?

Prescriptions are renewed during normal office hours. We ask that you call the office at least two (2) days prior for any refill requests. In some instances, we may require you to be seen in our office prior to medication renewal. Medication requests called in on Friday after 12 pm will NOT be renewed until Monday.

In Office: You can pay via cash, check or credit card (Mastercard or VISA) at either office location. 

Patient Portal:  Log onto the patient portal and click on the Billing tab to make a VISA or Mastercard payment.  If you need a login or need your password reset, please contact our office.
